The Short Answer
Guaranteed issue life insurance is a type of whole life policy that accepts every applicant regardless of health — no medical exam, no health questions, no possibility of denial. Coverage typically ranges from $5,000 to $25,000 with premiums between $30–$100/month depending on age. The trade-off is a 2–3 year graded death benefit period during which beneficiaries receive only a return of premiums plus interest if the insured passes away. It's designed as a last resort for people who cannot qualify for any other type of coverage — and for that purpose, it works remarkably well.
If you've been declined for life insurance — maybe due to serious health conditions, age, or a combination of both — you might think coverage is completely out of reach. Guaranteed issue life insurance exists specifically for people in your situation. It's not the cheapest or most generous option, but it guarantees you can leave something behind for the people you love. This guide covers exactly how it works, what it costs, who it's designed for, and when you should consider other options first.
For a broader overview of coverage options for people with health challenges, see our guide to life insurance with pre-existing conditions. For seniors exploring all available options, our seniors over 80 guide covers the full landscape.
The concept is simple: you apply, you're accepted. No medical exam. No health questionnaire. No blood tests. No prescription drug checks. The insurance company doesn't ask a single question about your health. If you meet the age requirements (typically 50–85) and can pay the premium, you're approved.
This sounds too good to be true — and there is a catch. Because the insurer takes on significantly more risk by accepting everyone, guaranteed issue policies come with important limitations:
Guaranteed issue is specifically designed for people who have been declined or would likely be declined for other types of coverage:

Understanding the three main types of life insurance underwriting helps you choose the right path:
| Feature | Guaranteed Issue | Simplified Issue | Fully Underwritten |
|---|---|---|---|
| Medical Exam | None | None | Required |
| Health Questions | None | 5–15 questions | Full application + records |
| Can You Be Denied? | No — guaranteed | Yes | Yes |
| Coverage Amounts | $5K–$25K | $10K–$500K | $50K–$10M+ |
| Graded Benefit? | Yes (2–3 years) | Sometimes | No |
| Relative Cost | Highest (2–4x) | Moderate (1.5–2x) | Lowest |
| Approval Speed | Instant | Same day–1 week | 2–6 weeks |
| Best For | Serious health issues | Moderate health issues | Good health |
Always try simplified issue before guaranteed issue. If you can answer "no" to the health questions on a simplified issue application, you'll get more coverage for less money with no graded benefit period.
Premiums are based almost entirely on age and gender. Here are approximate monthly rates for $10,000 in coverage:
| Age | Male (Monthly) | Female (Monthly) |
|---|---|---|
| 50 | $28–$35 | $22–$28 |
| 55 | $35–$45 | $28–$36 |
| 60 | $45–$58 | $36–$46 |
| 65 | $55–$72 | $44–$58 |
| 70 | $68–$90 | $55–$72 |
| 75 | $85–$115 | $70–$92 |
| 80 | $110–$145 | $90–$118 |
Not Sure Which Option Is Right for You?
Our brokers can help determine if you qualify for simplified issue first — potentially saving you 40–60% on premiums.
If the insured passes away from natural causes during the graded period, the beneficiary receives a return of all premiums paid plus interest (typically 10–20%), not the full death benefit. For example, if you paid $50/month for 18 months ($900 total), your beneficiary would receive approximately $900–$1,080.
Once you survive past the graded period, the full death benefit is in effect. A $15,000 policy pays the full $15,000 to your beneficiary.
Most guaranteed issue policies pay the full death benefit from day one if death results from an accident — the graded benefit only applies to natural causes.
